I have fallen into the category of "Taking my insurance company and their agents for granted!" Over the years I have been treated well and very satisfied. Many times I have thought of thanking all of you for the service I received, and didn't.
Today I contacted

authorization form.
We discussed it and she confirmed the information. She could have hung up and went on with her work, instead she read more regarding my call. She then called me back to make sure I had the correct information.
